Reading the chart. The Kaiser Family Foundation (KFF) Annual Employer Health Benefits Survey is the most authoritative source for U.S. employer-sponsored insurance costs. The 'family premium' is the total annual premium for a family plan — both the employer's contribution and the worker's contribution combined. KFF survey methodology has been consistent since 1999.
